Where has scope expanded in your work without anyone deciding it should?
Look for the pattern rather than the incident: a tool that started doing one thing and now does four; a document that began as a boundary and became a wish list; an approval that used to be required and quietly stopped being asked for.
Write honestly rather than tidily. The journal is continuous across the course — you will return to it. I read every entry and respond.
